- Names/addresses of employers for two years
- Most recent copies of pay stubs reflecting a minimum of 30 days of income
- Most recent W-2’s for two years
- Recent two years of tax returns
- If self-employed, year-to-date profit and loss statement, plus signed business returns for last two years
- Proof of pension income, if applicable
- Social Security and Disability payments, if applicable
- Dividend earnings, if applicable
